The Appropriations Committee on Health and Human Services heard presentations on the governor’s proposed fiscal year 2026-2027 budget for the health and human services agencies. Kendall Kelly outlined the overall HHS budget at $48.5 billion, with AHCA accounting for the largest share, and agency heads then highlighted major proposals for Medicaid behavioral health redesign, APD waiver enrollment and facility needs, DCF child welfare, opioid, and mental health investments, DOEA funding for Alzheimer’s, home care, and community services, DOH funding for cancer research, public health initiatives, and lab capacity, and VA funding for facility improvements, cybersecurity, and medication management.
Several members praised specific proposals, including increased reimbursement for private duty nursing, Alzheimer’s supports, and the Florida FIRST blood-in-ambulance initiative. Senators also questioned the proposed changes to the AIDS Drug Assistance Program (ADAP), with the Surgeon General explaining that the department expects a reduction in covered patients from about 30,000 to about 20,000 because of funding pressures tied to rebates, federal changes, and premium tax credit issues. Public testimony strongly criticized the ADAP changes, citing lack of transparency and warning that many patients could lose access to medications.
Other questions focused on the Office of Minority Health and Health Equity, DCF’s substance abuse and mental health data dashboard, Kids Care/CHIP expansion implementation, APD bed and facility planning, and the FX Medicaid technology project. DCF said about $7 million is set aside for the dashboard system, and AHCA said the governor’s budget includes $124.4 million for FX maintenance and continued module development, with $13.5 million to begin claims processing work. The committee did not take a substantive vote on the budget presentations and adjourned after questions and public testimony.

Several speakers emphasized that managed care and coordinated care can improve outcomes while reducing unnecessary utilization and costs.
Members asked about the impact of Medicare’s V28 changes, mobile cancer screening, urgent care versus emergency room billing, pediatric specialty access, complaint resolution, Black maternal mortality, provider shortages, network adequacy, and the use of AI in prior authorization. Witnesses said V28 has affected providers and revenue, UF Health’s mobile screening program is expanding beyond a few cancer types, and its urgent care model bills patients at the appropriate level rather than both urgent care and ER rates. Nemours said it reduced specialty wait times through scheduling changes, telemedicine, and registry tools, while AdventHealth described postpartum coordination and maternal heart programs to reduce maternal complications and mortality.
On complaints and access problems, panelists said their organizations use patient/member advocates, care managers, call centers, and escalation processes to resolve issues, and Sunshine Health specifically discussed a transportation complaint that was addressed with its vendor and the family. Sunshine Health also said it is not using AI for prior authorization, though it is exploring responsible uses elsewhere, and Florida Community Care said it is not using AI in utilization management. In closing, panelists identified workforce shortages, provider burnout, and high-cost drugs as the biggest ongoing challenges. The meeting ended with thanks to the panel and adjournment after Representative Brackett moved to rise, without objection.

Members then heard a presentation from Roger McCann of the Department for Community Based Services on Kentucky’s 2026-2027 Community Services Block Grant state plan. He explained that the federally funded grant, now about $12 million annually, is distributed through 23 local community action agencies that use flexible funds for locally identified needs such as housing, nutrition, transportation, Head Start, and domestic violence services. McCann said the plan is submitted every two years and that the agencies served about 264,000 Kentuckians and 130,000 families in 2024. Senator Meredith asked whether there was a central place to review local projects, and McCann said regular reports exist and could be shared with members.
The committee then approved the CSBG state plan by roll call vote. After that, members received an update on Kentucky’s state-designated domestic violence shelter programs from Angela Yanelli of ZeroV, Mary Foley of Maryman House Domestic Crisis Center, and Elizabeth Martin of the Center for Women and Families. The presenters described ZeroV’s role as the statewide domestic violence coalition, its 15 member programs across all area development districts, and its contract with the Cabinet for Health and Family Services to provide emergency shelter, supportive services, housing assistance, and batterers intervention programming. They emphasized that services are available 24/7 and are trauma-informed, with a strong focus on children and family support.
The domestic violence providers reported high demand and rising costs. ZeroV said its network served more than 14,000 adults and children in fiscal year 2025, including nearly 900 children in shelter and more than 500 in outreach, while Maryman House reported operating at 90-92% capacity, with 53 households waiting for emergency shelter and 63 more waiting for intake. Maryman House also described an 8-unit transitional housing complex and plans for a 48-unit apartment complex if tax credit funding is approved. The Center for Women and Families highlighted school-readiness support for children, tutoring, counseling, and family services, including back-to-school supplies and rights information for homeless students. No additional votes or formal actions were taken during the domestic violence presentations.

Testimony from BU law professor Jonathan Feingold argued that many DEI practices remain legally defensible after Students for Fair Admissions v. Harvard, and that the decision did not end all race-conscious or equity-oriented efforts. He said the Trump administration’s anti-DEI actions and funding threats were legally suspect and had created confusion and a chilling effect. Bahar Akman-in-Boden of the Hildreth Institute testified that proposed federal cuts to TRIO, Gear Up, Pell Grants, SEOG, work-study, and student loan programs would disproportionately harm low-income, first-generation, Black, Latino, and other underserved students, and urged the state to prepare hold-harmless and advising supports using Fair Share revenue.
Commissioner Noe Ortega described Massachusetts’ long history of equity in higher education and said the state has expanded aid, success programs, and early college, but still has work to do on attainment and completion. He said the state responded to SFFA by creating ACARE and continuing to defend equity practices, while also warning that federal disruptions and “dear colleague” letters have created uncertainty. In the second panel, state university leaders and campus officials said federal threats to Pell, DEI grants, Medicaid, and international student policies could affect access, campus operations, and the economy; they stressed that most state university graduates stay in Massachusetts and that institutions are continuing their equity practices despite federal pressure. Roxbury Community College’s president said RCC remains committed to open access and inclusion, noted enrollment growth, and said executive orders do not change existing law or the college’s obligations.

The committee then heard from Community Farm Alliance on Kentucky Double Dollars, Fresh Rx for Moms, and farmers market support programs. CFA reported expansion to roadside stands, more retail onboarding, seven new counties, and estimated economic and farmgate impacts, emphasizing that state funding helps leverage federal and private dollars and stabilize local food access programs. No formal votes or legislative actions were taken beyond approving the May minutes.
